- Assurant Health Access: Fixed-benefit health plan that pays you or the health care provider a set amount for covered services no matter how much the service costs. There is no deductible with this plan. Assurant Health offers three fixed-benefit health plans to meet your needs. You can choose to add dental, critical illness or accident insurance to your fixed-benefit plan. The lifetime maximum benefit ranges from $1 million to $3 million.
- Assurant Major Medical: This health plan is more traditional. Policy holders choose a deductible, co-insurance amount and co-pay. Policy premiums will vary based on the details chosen for your plan. There are two major medical health insurance policies available from Assurant Health. There is no maximum lifetime benefit.

Just trying to find a claim form to print for my son’s recent medical expenses. Can’t ever get anyone on the phone & there is a disconnect between you & the doctors as to out of pocket expenses. Since you are so unavailable, I’m sending you these receipts & ask that you give us credit toward the deductible (which is right & lawful). Sorry, but just can’t do anything more with the lack of info & resources you post for policy holders.
My comment is same as ladies above. I have tried calling several times. Cannot find a claim form to submit. When previously I asked could I go to Walgreens for vaccine, I was told to just send receipts. SO that is what I am going to do. Wish me luck.

I agree with all the above comments. You should make filing a claim more readily available. How hard can it be to fill out an online form, attach doctors bills and then you process. Come on, Assurant, get up with the times and get an online form!! Your process takes too long is too cumbersome. Snail mail, really????
