Filing the Claim With Cigna Insurance Provider

Most medical providers will file a claim for services rendered to Cigna Insurance, but it you need to file your own claim; you’ll need to fill out the Member Claim Form available at http://www.cigna.com/assets/docs/privacy-notices-and-forms/forms_medical_claim_form.pdf. The form must be printed, filled out and returned to the claim address printed on the back of your insurance card.

About A Cigna Insurance Health Plan

If you’ve purchased your Cigna Insurance health plan through your employer, payments are deducted from your paycheck weekly, biweekly or monthly, depending on your pay scale. If you purchased a health care plan individually, you will be sent a bill on the schedule established during the underwriting process. Coverage Details

  • Open Access: You have open access to doctors in and out of the network. You do not need a referral to make an appointment with a specialist. A higher deductible means a lower monthly premium. Payments range from 80% to 100% after meeting deductible.
  • Open Access Value: Same as the open access plan, but the maximum payment after meeting the deductible is 70%.
  • Health Savings Plan: After meeting a higher deductible, Cigna pays 100% of most in network costs and 70% of out of network costs.
