Fidelity Insurance Claim

Filing The Claim With Fidelity Insurance Provider

The beneficiary of any Fidelity life insurance policy can file a claim with the company by mail. Claim forms are available by state, from the official website here: Claims must be notarized, then submitted by fax or US mail.

Fidelity is one of the leading life insurance providers in the US. Established in 1896, it is also one of the oldest. As a company, Fidelity prides itself on innovation and customer service, and their online underwriting system is unique within the industry. Their aim with this is to ultimately speed up the underwriting process, and they claim to “virtually eliminate” tedious medical examinations and physical tests commonly associated with picking up a life insurance policy.

About A Fidelity Life Plan

Fidelity life insurance premiums are billed 21 days ahead of the invoice due date. A reminder notice will also be mailed 10 days after the premium’s due date.

Coverage Details

Fidelity offers a range of life insurance products:

  • Rapid Decision Term:No doctor exams, just an online questionnaire, making this an expedited option. The official Fidelity website promises you’ll get coverage in days, not weeks. This is a level death benefit policy, up to age 95. It is convertible during the initial 10 years of coverage, or until you reach the age of 65.
  • Rapid Decision Universal Life:Like rapid decision term insurance, this policy can be obtained by answering an online questionnaire. It is available to those aged 16-75. It also allows for opt-in riders such as Dependent Child coverage or Waiver of Premium.
  • Graded Death Benefit Term & Whole Life:This is the option for anyone with an existing health condition. Up to $250,000 in coverage is available, with maturity in year 3. Again, there is a simple online questionnaire to determine eligibility.
  • Accidental Death Benefit: This is the most basic life insurance option offered by Fidelity. Premiums can average about $10/ month, and the claim scale ranges from $50,000 to $500,000. Full benefits extend until age 70, after which only 50% of the policy is honored until age 80. Suicide, alcohol or drug related accidents, and military service all void this policy.

It is not possible to sign up for Fidelity life insurance by mail. Instead, you must go through an authorized Fidelity agent. For assistance in contacting a local agent, you can email
