Lifewise offers four main health insurance plans: WiseValue Plus, WiseValue Plus RX, WiseOptimum and Wise HSA. Dental insurance is available as an add-on to your Lifewise health insurance coverage. All Lifewise health insurance plans come with supplemental accident benefit coverage as part of the premium.
- WiseValue Plus: First three doctor’s visits with copay only. Generic prescription medications are covered.
- WiseValue Plus RX: First four doctor’s visits and four specialist visits at copay only. Brand-name and generic prescription medications are covered.
- WiseOptimum: First four doctor’s visits and unlimited specialist visits at copay. Brand-name and generic prescriptions are covered in addition to vision and maternity coverage.
- WiseHSA: Designed to work in conjunction with an HSA plan to provide medical coverage after a deductible is met.
- Dental: You can add dental coverage to your Lifewise health insurance plan for an additional cost. Deductibles are based on age with the highest deductibles being paid by customers 65 years of age and older.
